Become a Client

How to Become a Client

​To participate in our program, clients must meet the following criteria:  
  • Must be over the age of 18
  • Must be uninsured or underinsured and financially disadvantaged as demonstrated by proof of household income
  • Must be able to provide proof of household income (tax return, Form W-2, paystub, proof of unemployment)
  • Be at or below 200% of the Federal Poverty Guidelines (refer to the chart to the right)​​
  • Be a resident of Mecklenburg County and able to see a mental health provider virtually or within Mecklenburg County​
Other Reduced Cost Therapy Options
Potential clients will need to complete the following steps:
  • Complete and submit the PFA Client Application Packet through the on-line application accessible via the button below.
    • If you are unable to submit the application on-line, contact our Executive Administrator at [email protected] to receive a paper application.
  • Provide proof of income (tax return, Form W-2, paystub, proof of unemployment).

Once your application and necessary documentation are received, your application will be reviewed to determine if you meet our qualifications and if your needs can be addressed through our program.

If you are accepted into the program, the Clinical Services Committee will work on matching you to a licensed mental health provider with the professional training and skillset to meet your individualized needs. If a licensed clinician is not available at the time of application, you will be placed on a waiting list. All notifications of application and enrollment status will be shared via email, text or phone call. Although we will work diligently to ensure you are connected with the right provider, the enrollment process can take up to a month. If your need is more urgent, please visit our Resource page for a list of organizations that can provide immediate assistance.
